Foundation for the Awakening Mind – Events Waiver and Release Agreement

1. Media Release Form and Agreement

By agreeing to these terms and conditions, I grant Foundation for the Awakening Mind permission to use my name, audio and visual recordings, visual images, and/or comments in media and publicity materials. In granting permission, I understand that the audio and visual recordings, images, and comments may appear in a variety of forms including but not limited to web, newsletters, CDs and DVDs, websites, brochures, books, YouTube videos, and podcasts.

I agree that the use of my audio and visual recordings and visual images recorded at the event, program, and/or residential stay is the exclusive property of Foundation for the Awakening Mind, and I waive all rights thereto.

2. Waiver and Release of Liability

By agreeing to these terms and conditions, I acknowledge that any support offered by the teachers and volunteers at the event, program, and/or residential stay, whether emotional or psychological, is not to be taken as medical advice in any way and is not a substitute for medical advice offered by my doctor. It is not recommended to come off any medications that I am currently taking while attending the event, program, and/or residential stay.

In choosing to participate in activities and sessions at the event, program, and/or residential stay, I fully accept and assume all risks whether before, during, or upon completion of the event, program, and/or residential stay. These risks include, without limitation: physical injury, sickness, disease, emotional and psychological distress, physical or emotional trauma, allergic reaction, death, injury due to physical and mental activities, contact with anyone inside or outside the event/program/stay, contact with animals or insects, construction materials, vehicles, tents or buildings and appliances, kitchen or electrical equipment; and/or injury due to contact with the natural environment including sticks, forests, and uneven rocky terrain; and/or injury due to the effects of weather including wind, hail, lightning, extreme temperatures, or other weather conditions.

I hereby acknowledge that I understand that the sessions, projects, and activities may be physically, psychologically, and emotionally stressful, and I am participating in the event, program, and/or residential stay voluntarily. I acknowledge that it is my sole responsibility to refrain from participating in any event, program, and/or residential stay activities which may be stressful or harmful to my emotional health or well-being.

Knowing these facts and in consideration of my admission to and participation in the event, program, and/or residential stay, I release, waive, discharge, covenant not to sue, and agree to hold harmless the following individuals and entities, hereinafter referred to as “Released Parties”: Foundation for the Awakening Mind, its officials, teachers, volunteers, support people, employees, and representatives, from any claims, demands, and actions of any kind.

I also agree to hold harmless the Released Parties from any injury to persons, and/or damage to, or loss of personal property, which may hereafter accrue against such parties directly or indirectly arising out of or relating in any respect to participation in the event, program, and/or residential stay activities, either on or off the property, including any excursions that may be organized as part of the event, program, and/or residential stay’s off-site locations.

My waiver and release of all such claims, demands, and actions for which the Released Parties might otherwise be liable shall include, without limitation, any injury, damage, or loss to my person or property which may be:
(a) caused by any act, or failure to act, by the above-identified persons and entities; or
(b) sustained before, during, or after my participation in the event, program, and/or residential stay and/or any other event, program, and/or residential stay that I attend in the future organized by the Released Parties.

I acknowledge that I may be asked to leave the event, program, and/or residential stay for any reason by the organizers without recourse.

I have had an opportunity to ask questions and obtain information regarding the event, program, and/or residential stay to my satisfaction. I am aware that this event, program, and/or residential stay is based solely on the teachings of A Course in Miracles and David Hoffmeister.

I subjectively understand the risks of my participation in the event, program, and/or residential stay, and knowing and appreciating these risks, I voluntarily choose to participate assuming all risks due to my participation. I warrant to the Released Parties that I have no physical, emotional, or psychological condition which would keep me from participating in and enjoying the benefits of the event or which would cause me injuries or harm.

I acknowledge and agree that the Released Parties are not responsible for providing any security while at the event, program, and/or residential stay.

By agreeing to these terms and conditions, I certify that I have read this document, and I fully understand its content. I am aware that this is a release of liability and a contract, and I agree to it of my own free will.

3. Refund Policy

All payments are non-refundable.
